COULD be... but I recall a patient in H-town (not Houston) who was sent to the hospital from a nursing home... was essentially direct-admitted to the hospitalist... Died before he was seen by anyone... ED did the rapid...

Maybe its better to say that there is nothing nefarious about the practices... but they can certainly be interpreted or used in nefarious ways

you don't delay signing a COD in order to keep the death from being reported properly to push a narrative... and states didn't decide how to report in order to manipulate the numbers

Well technically, the COD should be assigned and signed within 72 hours by law in CA. It took 3 weeks because nobody would take responsibility and kept trying to make someone else do it. You know how doctors can be.
Even the cause of death can be manipulated by the doctor. Without an autopsy to confirm the actual cause, the cause is the opinion of the doctor that is doing the certification. And this opinion can be altered to fit a provider's narrative.
