Here's how.

When the shots started, Trump was facing almost directly to his right. The bullet that hit him grazed his right ear.

That means the bullet(s) could have flown across the stage from left to right, grazed Trump, then exited across the arena without hitting anybody behind him. The bullet(s) in that scenario would have passed between Trump and those behind him.

Fox now reporting that at least one spectator behind Trump was hit and is badly injured, had to be tended to in the stands before being removed.
