A weapons scientist made the point today that we saw an example of what happens when a Cold War era system meets 21st century technology. There have been a lot of close calls in the past that never got out publicly. This wasn't a close call, but turned into a public relations nightmare for an entire state. In a sense, it's the scariest event for a population center since the Cuban Missile Crisis, even though there actually was zero danger whatsoever outside of people doing really stupid things as they attempted to take cover.

I can about predict the solutions.... Additional manuals and educational videos on button pushing procedures and retraining modules set up...redundancy's set up with at least one new employee to overlook the button pusher...CCTV cameras installed and video recording of the button pushing with an uplink to the supervisory bureaucrat. New national conference in Las Vegas for the supervisors of button pushers initiated. In other words...MORE bureaucratic nonsense.

My solution....Install a $2 hard cover over that button that has to be removed in order to push it.

I can easily see this as a stupid mistake regardless of a confirmation step.
The vast majority of problems like this are simple human error.

What I don't want to see is some attempt to idiot proof everything such that a real threat warning is missed by over complicating a simple decision. For example, some Hawaiian officials mentioned a change to require at least two people to initiate the warning. So what happens when one of them has to take a piss? Simple things cause the problems not some conspiracy.

We have a "clipped" flip over button cover on one of our machines at work. If pushed?...It shuts down the machine in a manner that destroys the product currently in the machine but essentially saves critical components from overheating and damaging the machine. The operator has to pull the clip and flip over the cover to push this button. Something like this is all they need. No need for additional bureaucratic involvement. The simplest and most cost effective solution is generally one using common sense solutions.

Ill bet right now...The answer to this will costly and unnecessary. That is how government grows. It knows no other way than to expand beyond its original scope to solve even the simplest problems.
