Only thing wrong I see is that they say B'ham-Hoover - how much is Hoover skewing the per capita income vs the other cities on the list or are they doing the same thing in those cities?

Hoover is wealthier than B'ham, but the main per capita income bounce comes from the cities between them.

Fifty years ago, there were several newspaper articles critical of the metro area's wealthy who invested in many places, but not inside B'ham. If that trend has been reversed, it can be a powerful stimulus to future growth for the whole metro area.

these are for MSA's not city limits. Hoover is large enough population wise that it is included in the name of the MSA for the greater Birmingham area. You don't have to live within the city limits to have an impact on the area. I am sure a substantial portion of the donations for UAB and other public private projects in the city limits of Birmingham came from people living outside the city limits.

Fifty years ago there was no "UAB" to which to contribute, and Hoover just recently celebrated its 40th birthday. If the investment trend is indeed showing greater willingness by local wealth to invest locally (not just UAB medical), that is a good thing.
