Because different facilities have different requirements for equipment and training. Part of the reason an ER is so expensive is that it has access and staff available to do almost literally anything that might be needed to save a life in the event of an emergency.. while an urgent care only has a very limited set of equipment... almost none of which are designed to facilitate 'life saving', other than a phone to call an ambulance to get them to a hospital where the ER will save them... unless those minutes make a difference.

Lots of specialists haven't done the things that might be required to save a life since medical school... and then, only under close supervision and in a lower stress environment. There's a reason they specialize
