I don't think there will be any states that turn down any of the vaccines. I'm not even sure they will have to formally make a request. It's now public information that the US, at the country level, has already ordered a bunch, and other large countries are already putting in orders.

"UK officials have scrambled to secure 5m doses of a breakthrough coronavirus vaccine within four hours of early results, amid fears that Britain had missed out on supplies.

Interim findings from the US biotech firm Moderna – whose trials included high-risk and elderly people – suggested its vaccine was 95% effective. It is more expensive than the Pfizer/BioNTech version announced last week but can potentially be stored and transported more easily.

messengerRNA (or mRNA, the basis of this vaccine), in lay term, is used to DNA instructions on how to produces proteins/cells/etc. In the case of this vaccine, the mRNA being developed exclusively produces the viral protein strains when encoded...now most viral proteins are effectively "dead" and do not have the ability to self-replicate, so this, in theory, is perfect fine, all I'm saying is we're operating in "theory" territory, and as one of my senior faculty always says, "Life always has a way of making 'in theory' irrelevant."

All of this is to say that I'm not at all skeptical with the immediate results and will absolutely be in line should these get approval, just that we simply do not know the long term effects of having synthetic mRNA activated proteins in our bodies, and more specifically, if giving the "instructions" to our DNA will allow them to continuously produce these proteins as there is no "off-switch" outside of our immune response and the expectation that our body's cells will know to stop producing these proteins.

The mRNA vaccines do technically prompt your body produce virus protein. That said, the proteins don't have the ability to infect your cells. It's similar to how we inject dead flu cells to develop flu immunity before the body is faced with active viral cells.

What do you mean "there's no off switch?" Vaccine contains only a finite amount of mRNA. There's no ongoing replication in your body. When the vaccine is cleared it's cleared and that's the end of it. Honestly I don't think you know what you're talkin about.

Yep, a small portion of mRNA that codes for the receptor binding domain on the spike protein of the virus (the part that actually sticks to your cell when being infected) - that small portion of mRNA that does nothing by itself is encased in a nano lipid particle (think small small small oil droplet). Since our cells are made of lipid on the outside - imagine oil droplets in water and how they interact with each other. So the nanoparticles fuse with our cell membranes and that tiny portion of mRNA is introduced into the cell where it's turned into binding domain proteins, not replicated, by our cells ribosomes. Since it's recognized as foreign - specialized molecules called MHC will grab it and displayed on the cell surface. From there our immune system takes over, sees the Protein that's been produced, and start producing antibodies against it. That is a nutshell is how it works.

I'm a Civil Engineer, all that I said is paraphrased from a couple of conversations I've had since these announcements with a member of faculty in our Bioengineering Department that works specifically on cellular response research. The "no off-switch" comment is probably oversimplified, but essentially what he is worried about is that a synthetic mRNA may or may not propagate in the body past the immuno-response, which would mean that we may be giving our cells a permanent means with which to replicate, again, some terminology may be messed up, as biology was never my strong suit (I could talk till I'm blue in the face about materials science and structural metallurgy). To be clear, he's not against them, and will get them same as me when they're available, he is just skeptical about the long-term impact.

Scientists at St. Jude Children’s Research Hospital have identified two proteins that may be linked to the inflammatory storm that causes lung damage, organ failure and often death in COVID patients.

Drugs already approved for other diseases, including Krohn’s disease, could be repurposed to fight the condition.

Astrazeneca's phase 3 trials are published. This is different than mRNA vaccines. Here a chimpanzee adenovirus is used as the vector delivering the viruses spike protein RNA. There were two types of dosing here. High-dose followed by high dose and a low dose followed by a high dose. The last (low-high) was 90% effective so I'm sure that's what they'll use --- plus it allows more people to be vaccinated (not as much vaccine used). Also they tested for asymptomatic infections PCR testing the participants each week. Pfizer and moderna did not do this. Here, in the vaccinated arm there were 30 total infections, 27 in high-high dose and 3 in low-high dose - zero severe infections and zero infections requiring hospitalization. We have another winner :-)

Actually I regret posting that gripe. It's my understanding they have thousands of pages of dense, highly technical reading. Since these vaccines will likely be given to billions of people - it's best to get it right on the front end.

It should be ready by around the end of the year, pending regulatory approval. The availability in the USA, however, will likely take longer due to need to alter trial for half dose/full dose method.

Our British cousins developed the shot.

Here are some additional facts per the WSJ:

1. Half dose followed by a full dose = 90% effective while two full doses were 62% effective.
2. Distribution does not require subfreezing temps
3. Astrazeneca does not intend to make a profit during the pandemic
4. Cost is about $3 to $5 per shot
5. They intend to distribute to developing countries
6. Manufacturing is a global effort

As an aside, the better efficacy of the low dose/high dose over high dose/high dose is counterintuitive. It is a reminder that common sense (e.g., source masking is very effective at preventing spread) cannot assumed to be correct. To the best of my knowledge, there is no randomized, controlled study finding source mask use beneficial.

However, I wish to emphasize I wear a KN-95 mask in appropriate situations because the weight of evidence to date supports its use and there is little risk assuming proper handling.
