Some states created their own exchanges while some refused forcing the feds to cover those exchanges. TN was one of the states that refused to create their own exchange.

No.
"Exchanges" are simply a Trivago for health insurance Co's and plans, you can view them all in one place, compare side by side, price, "benefits"
or whatever else.
The Medicaid expansion was more similar, or almost identical, to the crack appeal of the tobacco "settlement" from years ago. Feds, (read zerO care) offering a 2-3 year cover for the expansion at something like 80-90%, then it gets fazed out and the State that smoked the crack is now on the hook for that expansion.
Total. Utter. Complete. Dumbassery for any of those States that decided to do so, simply another unfunded Federal mandate. The white paper for Kentucky was actually posted here at the time, so it's probably still around, go take a look as an example.
No doubt the thinking was "We'll do this now, and of course the Feds will still continue funding cause once there's an entitlement, we've not ever gotten rid of a one.
