Tried to Google it. First couple pages results mostly New York & Cuomo saying they're still too dangerous.

I assume most are still closed?

They're still closed here in MI.
I think they're open in Memphis, not sure. My company has a gym in the back of our building that we all use.
My gym has been open since the NC legislature allowed gyms to be open for members with medical conditions in which one needs gym access back in July. Nice slight of hand IMO by the legislature...it allowed pretty much everyone back in with that clause. We social distance and limit the number of people in the gym. All the machines are over 6 feet apart and the showers and towel service is halted for now. My functional fitness class takes uber care in cleaning the hell out of everything before and after class. We do not wear masks..period. There has not been a single case of covid in our gym since reopening despite our county having one of the highest rates per population in the state.
Yes gyms in Ohio opened back up May 21st.
In Georgia they opened a few weeks ago. But its usually limited.
Yes they have been open since mid June
Been open since May here. South Alabama area.
Been open since May. Can't drink out of the drink fountain. Can only fill your cup.

Been open since early May in CT.
In these open gym states, has any outbreak been linked back to a gym?
yes
Opened here.. Although the 24 hour gyms now open at 7 and close at 10.. So does me no good. Ended my membership

Not that I'm aware of.
Yep, open in Arlington TN
They are open in EP, not that I'm going but I see their signs saying that they're cleaner than ever before. So, yes, I guess.

no. no no! According to the Dem establishment and the MSM, you're all dead now. You'll find this out when the Dems mail in your ballots for you this November.
Texas gyms are allowed opened under the following conditions:

1. 50% occupancy (i.e. for 'weight gyms' every other bay is not allowed to be used, for those like Orange Theory 50% class size)
2. mandatory cleaning of equipment after each individual use
3. mask on when you enter, mask on when you leave
4. no shower facilities permitted.

Many have instituted 'cant enter until' items by themselves --- a) temperature check on entering; b) limit of personal items to only keys
