No they shot at the autistic guy with a toy truck and missed, hitting the therapist.

My first thoughts, as well. Do we know if the shot to the caretaker's leg was a direct hit or perhaps shrapnel from a missed shot? A direct hit from an AR-15 isn't going to feel like a "mosquito bite".
