If someon smokes a lo of cigarettes daily and gets lung cancer, if someone drinks a 1/5th of liquor a day and has liver damage, if someone eats yons of sugar/carbs and gets diabetes, if someone eats 5000 calories a day and high cholesterol diet and has a heart attack, should we all have to pay money for medical bills?

I think we should establish colonies, something like what they used to do for lepers, we should take all the those folks you mention(smokers with lung cancer, alcoholics with liver disease, fat people with heart disease, etc) and just put them in one of these colonies and let them die.

Or, if you choose to live in an urban area with some level of air pollution?

The list is endless. It is all their choice to do these things. No one forced them to do any of these activities.
