Clarification Needed...
1. Do those who completed and sent in the Football Foundation pledge form have to submit another one (the UAB Pledge form)?
2. Are the Football Foundation pledge forms to be turned over to UAB?
3. Are new donors to complete the UAB Pledge form and/or the Football Foundation pledge form(s)? The original Football Foundation pledge form is still being passed around twitterdom.
4. The UAB Pledge form has no contact information. How is this information to be passed along?

According to Brad Hardekopf, they need the new forms for it to count. They will do their best to convert the FF forms over, but if you want to be CERTAIN your pledge counts send in the new form.
Item 4= a good question.

1. Justin is trying to get UAB to accept the previous form, but going forward, we need to use the new form. We will hear from Justin when he knows if the old form is acceptable. My advice - re-do it on the new form and make note of that when you send it in.
2. Not sure. See above. That's all I know for now.
3. New pledges should be done on the new 3-page, cumbersome form. Nothing is ever easy at UAB. smh.
4. Write it in at the bottom below the signature. Make the pledges, and make them quickly!

I called Brad and talked with him this afternoon. I asked the same question. He said it was fine to add to the form. I added my address under my name on the last page. I also asked about the date for the first payment. He said I could write in "when football returns".

1. Yes
2. Yes--the UAB Athletics Department will use those forms to contact those people to get them to fill out/sign an official UAB form.
3. The UAB form, although you might want to do both so that they get your contact info. However, the UAB Campaign form is the only one that will count towards the donation number.
4. This is an open question
