Terminal Degree
Just throwing some news out there for some of you that know me,
I finally finished my doctorate. Just presented at our national conference in Las Vegas (just got home today).

My presentation and manuscript was, "A Preliminary Evaluation of a Mathematical Model to Estimate Patient Blood Volume." Working to get it published now. Pretty cool- stats and boring stuff aside, we were able to predict patient blood volume within a 1 to 5% range as compared to actual measure blood volume by the Daxor BVA-100 (based on time-decay of patient blood sample).

Based on your work, would it help dial-in dosages of something like EPO to increase blood production to being within normal limits?

Pffffft, Lance had that figured out years ago.

Hahahaha...

To answer the question, though... Not at this time. The importance of this study is that it is a relatively simple concept that can be used in daily clinical anesthesia practice to help guide our fluid therapy during and immediately after surgery. Currently, we use outdated and non evidence-based formulas originally penned in 1957 that make theoretical sense, but have never been proven, or we use what is termed "goal-directed" fluid therapy that relies on surrogate indicators, is not risk averse, and is expensive. This method requires a baseline hematocrit, a fluid challenge with a known quantity of fluid, a repeated hematocrit value, the patient's ideal blood volume, and our formula. It's actually very simple.
Now, my mentor and I are writing this up for a grant for a prospective study.

Thanks to all for the congrats.

RE: Terminal Degree
So, you're basically trying to find out fluid levels based on hematocrit levels. Are you taking urine specific gravity levels into account or is it just dealing with current fluid levels not voided?

And here I was only thinking how great it is that USAFBF successfully finished the difficult program he started. Congratulations young man and I hope you get the results you desire.

Who, as it turns out, did indeed derail this thread?

Dealing with blood volume within a closed system. Hct doesn't deal with urine output nor sp. gravity. This is a basically a snapshot in time of the the patient's blood volume at the point of the tests (Hct's #1 and #2), with the goal of maintaining that level as baseline- an assumed euvolemia as the goal of replacing lost fluids by either surgical blood loss, urine voided, or evaporation from the patient's cavity exposed to air during the surgical procedure (referred to as insensible losses). So, at this point, urine sp. gravity plays no part in this early work. However, we have in our brain-dump (the mind map we used to guide this thing) further intangibles that will also play to the complexity of this project for future study, and that includes other fluid losses as well and could involve using urine sp. gravity as another means for establishing a reference point of patient baseline and/or their euvolmeic/volume status.

Thank you very much... I know many of us on here like to give you a hard time (some more than others), but truly you are a good sport. Thanks very much, especially for referring to me as a young man!
