I wanna say the Sacramento Kings..
I think they were Rodchester Royals, [EDIT] Cincinnati Royals (9p 10/25) [/EDIT] , Kansas City Kings, then Sacramento Kings.

I think there are several two city teams
Minneapolis/LA lakers
Vancouver/ Memphis Grizzlies
Charlotte/New Orleans Hornets

Well, it looks like the Kings are tied with the Hawks, who started out as the Tri-City blackhawks (the tri-city area was in Illinois), they then moved to Milwaukee, St. Louis and finally Atlanta.
The Bullets/Wizards have played in three cities, Chicago, Baltimore and DC, and they have changed their name a total of 5 times.
Clippers are another team you left off, Buffalo, to San Diego, to LA.
There are a few other teams that have moved/changed names.

Depends on how you want to count answers
Sacramento had Rodchester, Cincinnati, Kansas City ( & Kansas City-Omaha), and Sacramento
AND
Atlanta had Tri-City, Miluakee, St Louis, and Atlanta
So either Terpy and I tied or since Terpy said Hawks and Kings are tied he gets both.
B/c noone has given a team that has gone to 5 cities. The 3s and 2s city teams don't count... just good trivial pursuit type stuff.
