On behalf of the ATF / UAB I am announcing an updated Pledge Form for everyone to fill out. If you have already made a pledge YOU NEED TO STILL FILL THIS OUT!!!!!! yes, it is a hoop they are making us jump through but they CAN. Fill it out for the same amount (or more) that you originally did. I will be e-mailing these out shortly to everyone that sent a pledge for to me. If you don't get one please PM or e-mail me. E-mail it to Brad or any of us taking the forms and we will get it to Brad.

See announcement:

In an effort to fulfill the charge given to the UAB Athletics Assessment Task Force to conduct an independent review of the sustainability of football, bowling and rifle programs, the subcommittee of Public/Private Support, led by Champion Club members Don Hire and Justin Craft, will continue to be assessing donor interest over a five year period. As discussed at last night’s subcommittee meeting, these five year pledges will indicate the level of support necessary. This support would be contingent on the sports returning.

Attached is the updated form needed to make your pledge. Please print the form, fill it out with your pledge information, and (preferably) scan it back to me. If you choose, to mail or drop off, see below for the Blazer Boosters address.

Ullman 339
1720 2nd Ave South
Birmingham, AL 35294-3350

If you or one of your personal contacts has filled out the previous form, please encourage them to resubmit on this attached form. Stay tuned for more details and please let me or someone on the Blazer Boosters staff know if you have any questions or if we can be of assistance in any way.

Also, if you know of others who might be willing to make a pledge, please feel free to pass this form along. This needs to be done as quickly as possible to ensure each pledge is counted in the CSS report.

Thanks again, and Go Blazers!

Just printed mine out and will get it back this afternoon
Where is the form? If you need a place to host it online send it to me and I'll give you a link
Got it. let me know if you can't read it. Its in the title post.
Ok, I see the attachment now. Filling it out and sending it in today.
I don't think they can deny it now.
Suggestion: this needs to be uploaded as a universally downloadable link. It will be much easier to distribute across social media.
Can someone do that and post it? I am now traveling. I got a doc up there too. word
On it, one sec
It looks like the University's gift agreement doesn't have the payment plan options that the FF form had. Just an observation I've seen others make.

I'm re-submitting my pledge, but just want to clarify before I carry the message to far. So, the only forms that will be accepted now are the new gift agreement forms? And anyone that filled out a FF pledge form needs to also fill out the new form? If those that filled out the previous form don't also fill out the new form will those pledges not be counted?

Correct. If you want your pledge counted fill out new form.

E-mail it to Brad. (Or any of us)
After reading this form I'm still concerned about its legitimacy, since it's a contract and should be signed by everyone involved in person. It also collects zero contact or personal information, so I'm not sure how the hundreds of these things will be followed up on.

I'm still filling it out and sending it in but I hope someone at some point starts thinking this through before we have to fill out a fourth or fifth form.
Fill them out and be counted people!!!!

I agree. The only place you put any information about yourself is your name on the last page. No address, no phone, no email, etc. How will they know to get in touch with potential donors?
Also, old FF pledge form still being "passed around" in twitteerverse...
Someone direct them to this form in twitterverse and Facebook.
New Form Sent
So is the UFF pledge form I mailed in null & void now? The forms Attalla post both look to be same form & seem to be in the Campaign for UAB rather than separate from Athletics giving (other than you can specify fb as mentioned last night). I thought the Campaign for UAB didn't have athletics mentioned in it? Red Flag???
